IEEE Event Participant COVID-19 Disclosure, Waiver and Release of Liability

In-person participation at the 2021 IEEE International Conference on Image Processing located in Anchorage, Alaska, USA (“Event”) will carry risks, such as exposure to COVID-19. While IEEE and its service providers are committed to take prudent steps to follow all rules, regulations and authoritative guidelines regarding health and safety practices, the risk of serious illness and death still exists.

Maintaining the safety of Event participants remains a priority and requires a personal commitment by everyone in attendance to abide by health-and-safety-beneficial conduct while in attendance.

As a condition of my in-person attendance at the Event, I AGREE that:

• I will adhere to all Event, Event venue, Event lodging and governmental rules and regulations.
• I will not attend the event in person if I am: (i) not feeling well or have experienced any symptoms associated with COVID-19, including fever, cough, and shortness of breath, among others within the 14 days prior to the start of the Event; or (ii) I have, or believe that I have, been exposed to a confirmed or suspected case of COVID-19 within 14 days prior to the start of the Event; or (iii) I have been diagnosed with COVID-19 and am not yet cleared as non-contagious by state or local public health authorities
• During and after my in-person participation in the Event, to monitor myself for signs and symptoms of COVID-19 and to contact IEEE at eert@ieee.org or if I am diagnosed with COVID-19 within 14 days after attending the Event
• I voluntarily elect to participate in the Event in-person with knowledge of the dangers involved of possible exposure to and illness from infectious diseases including, but not limited to, COVID-19
• COVID-19 is an extremely contagious disease that can lead to severe illness and death. I acknowledge that participation in the event and its and activities brings risk of exposure to COVID-19 and I do hereby freely accept and assume any and all COVID-19 related risks.

On behalf of myself, my personal representatives, heirs, next of kin, successors and assigns, I forever waive, release, hold harmless and discharge IEEE, its officers, directors, employees, members, volunteers, technical sponsors and other participants from any and all negligence and liability for my death, disability, personal injury or property loss of any nature as a direct or indirect result of my participation in the Event.

I understand IEEE will not (i) indemnify me for any health risk or economic or other loss resulting from exposure to COVID-19 during travel and participation in the Event; and (ii) will not reimburse me for expenses incurred during mandatory or voluntary quarantines associated with the Event or for any quarantine related costs.

I understand and agree that the law of New Jersey, U.S.A. will apply to this contract.

I affirm that I am: (i) at least 18 years of age; (ii) freely signing this form; and (iii) fully aware that by signing this form I am giving up legal rights and/or remedies which may otherwise be available to me regarding any losses I may sustain as a result of my participation in the Event.
